Patient Eligibility

Requirements to determine eligibility: HOPE Dental Clinic serves adults in the CSRA who do NOT have dental insurance and meets one or more of the following criteria:

• Individual or spouse must be employed* (at least part time) OR
• Individual must be enrolled* in college or technical college OR
• Individual is a senior (60 or over) OR

• If unemployed, individual is actively seeking employment (must show proof of employment within the prior 6 months or less)

Identification:  Must bring picture id and social security card to put in patient file.

Proof of income:  Income must include all persons residing in the household. Individuals should furnish accurate information in regard to total earnings after deductions, of all household members. Please bring all the following that apply as proof of income.

• Previous year's tax return
• Two current pay stubs (patient and spouse)
• Last paystub (if unemployed) showing employment within last six months
• Social Security Administration Award Letter
• Information indicating patient is receiving Social Security Disability, Pension, Retirement, Veterans Administration Benefits or any other sources of income

Fees for HOPE Dental Clinic will be based on household income, number of dependents and type of dental procedures. We currently only offer fillings, extractions, cleanings and crowns.

There are NO EMERGENCY SERVICES available at this time.
